Bladder dysfunction is the one of the serious postoperative complication after surgery<br>for rectal cancer, with a reported incidence of 7.5 to 69 percent. The pelvic autonomic<br>nerve preserving procedure(PANP) is the newly devolped operative procedure that<br>preserves the autonomic nerves responsible for voiding function as well as sexual<br>function. To evaluate the significance of the PANP on bladder function after surgery,<br>urodynamic study was carried on both pre-and post-operation. 33 patients with rectal<br>cancer who underwent operation at Kyung Hee University Hospital from the January<br>1987 to June 1993 were reviewed. The patients were divided two groups according to<br>the operative procedures, 13 patients with conventional operative procedures(conventional<br>group) and 20 patients with rho autonomic nerve preservation operation(ANP group).<br>The time of the first-voiding was staststically significant as 2.55?.48 days at the ANP<br>group with compared to 5.92?.25 days at the conventional group(p<0.05). About urine<br>flow rate, the ANP group showed no difference between pre- and postoperatively, but<br>conventional group showed significant decrease in flow rate postoperatively(p<0.05). And<br>there were no differences at the first desire volume and detrusor pressure, the maximal<br>desire volume and detrusor pressure between the ANP and conventional group, however<br>the urinary retention revealed clinically at 50% of the conventional group. In conclusion,<br>the autonomic nerve preservation operation is the excellent operative procedures of<br>choice for rectal cancer in view of postoperative voiding function.<br>
