Abstract
This report is a clinical review of 111 patients over the age of sixty who underwent
appendectomy due to acute appendicitis in Dept. of General Surgery of Dong-Guk
University College of Medicine from January, 1987 to April, 1992.
The results were as follows:
1) of 1506 patients with acute appendicitis, 111 cases(7.37%) were acted over 60 years
old.
2) Male to female sex ratio was 1 : 1.92.
3) The duration of symptoms varied greatly, and was related to outcome. The mean
duration was 1.81 days.
4) On admission, the most common symptoms and physical findings were right lower
quadrant pain in 66.7%, nausea in 56.7%, right lower Quadrant tenderness in 79.2%,
rebound tenderness in 64.8%.
5) Leukocytosis(10,000/mm

) on admission was present in 68.5% of
patient. But, 54.1% patient had a temperature below 37¡É.
6) 61 cases(55%) were Perforated and 6 cases revealed generalized peritonitis.
7) The abdomen was entered mainly through Rockey-Davis incision (66.7%) and
McBurney incision(11.7%), right lower paramedian incision(15.3%). Operation method was
appendectomy in 61.3%, appendectomy with drainage in 31.5%.
8) The postoperative compication was occurred in 31.5% of patients and the most
common complication was immediate postoperative Partial ileum(15 cases) and wound
infection(14 cases)
9) 63 cases(56.8%) were accompanied with concomitant diseases.
10) The average hospital day was 9.6 day. (8.2 day in nonperforated cases, 10.8 day
in perforated cases).
